You don't need to have the words figured out before you start. Here is what the path looks like.

    Request a visit, or book online

    New patients start with the request form below; existing patients can self-schedule in the patient portal. Please book at least 24 hours ahead.

  2. Marker two

    Forms & screeners in the portal

    Intake, financial policy, privacy notice and consents, plus the PHQ-9, GAD-7 and PCL-5. Perinatal and postpartum patients also complete the Edinburgh scale.

  3. Marker three

    Your first visit

    In person in Monument or by secure video anywhere in Colorado. A real conversation about your history, your goals and your options — not a checklist.

    Follow-ups & recall reminders

    Shannon sets a follow-up recall and you get a reminder about a week before it is due. You can also self-schedule right after a visit.

Shannon Wright is a board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner, which means hormones and mood get looked at together instead of bounced between offices. TrailMind Health & Wellness is her solo practice in Monument, just north of Colorado Springs in the Tri-Lakes area, with video visits for patients anywhere in Colorado.

A card on file is required for every patient, and copays are collected at the visit. Details are in the financial policy you'll review before your first visit.
A few things to know

Before we meet.

What is the first visit like?

It is a conversation about what brings you in, your history and what you hope will change. There is time for questions before we talk about options. You can come to the Woodmoor office in Monument or meet by secure video from anywhere in Colorado.

What do I complete before we meet?

Through the patient portal: the new-patient intake, financial policy, privacy notice, consent to treat and telehealth consent, plus the PHQ-9, GAD-7 and PCL-5 screeners. If you are pregnant or postpartum you will also complete the Edinburgh Postnatal Depression Scale. Follow-ups usually repeat the PHQ-9 and GAD-7.

In person or by video?

Either. In-person visits are at 1864 Woodmoor Dr., Ste 214 in Monument. Video visits are available to patients anywhere in Colorado and work well for most follow-ups.

What if I'm running late or need to cancel?

Please book and reschedule at least 24 hours in advance. There is a 10-minute grace period for late arrivals; after that the visit is rescheduled. No-show fees are described in the financial policy.

How do follow-ups work?

Shannon sets a follow-up recall at the end of each visit and the practice sends a reminder about a week before it is due. You can also self-schedule in the portal whenever it is convenient, and send refill requests and secure messages there.
